This form is for refills only. Please drop off new prescriptions at
the store or fax them to (248) 486-1906.
Please allow 24 hours for us to complete your compound
for pick up. (this does not include weekends or holidays)
For shipping, please allow a few days for the shipment
to reach you. (UPS Ground typically arrives the day after the shipment
is sent. Friday shipments arrive Monday. First Class mail typically
takes 3-7 days.)
Prescriptions shipped
first class mail via the United States Post Office are not trackable
and therefore are not replaced if lost in transit. If you have
a question about this policy please
call the pharmacybefore
requesting shipment.
Refrigerated medications are only shipped Monday through Thursday so
they will arrive the following day. Holidays may affect this schedule.
Holiday shipping: Please call to inquire about blackout
dates for shipping during any holiday. Please allow extra time for
shipping via the Post Office

schedule.
